If you are a physician who solely owns your practice or you are employed independently, please select the individual quote option. This will provide a pricing estimate for one physician only.
If your group practice is not currently insured by LSA, you are requesting a quote for two or more physicians and any additional providers who all work together at the same practice and operate as a group (i.e. sharing records, resources, responsibilities for patient care, etc.) and you're seeking a quote for the entire practice, please select the group quote option. This will provide an itemized breakdown of pricing for each physician, each allied health care professional (If applicable), and the entity (if eligible).
